Transcript Request

To request a transcript:

  1. Print out “Official Transcript Release” form.
  2. Fill in “Official Transcript Release” form and sign (Parent or Guardian must sign if student is not of legal age). 
  3. Fax it to the appropriate fax number for school attended.
